Printable attendance sign-in sheet for meetings

PTA meetings, club nights, and trainings still start with a sign-in sheet on the table by the door. Type your meeting details, pick how many rows you need, and print numbered rows for Name, Email, and Phone, with optional columns for first-time visitors, role, and a signature. We don't save what you type, and you don't need an account.

Optional. Prints above the meeting name.

From 1 to 200. Name, Email, and Phone always print.

Extra columns

A box newcomers can check.

A title, grade, company, or member type.

A wide box for a signed record.

Total present:

25 numbered rows with Name, Email, and Phone for Monthly General Meeting on Tuesday, November 10, 2026, from 6:30 PM to 8:00 PM.

Why put it on a sign-up sheet

One link replaces the sheet on the table. People sign up ahead with a name and email, no account, so you know who's coming before the meeting starts. SignUpSpark emails a reminder 24 hours and 1 hour before each slot's start time, and you can export the list to CSV.

Turn this into a sign-up sheet

Opens a sign-up sheet with 1 slot at your meeting time and 25 spots, one for each row, so people can sign up ahead. Change any of it before you publish.

How it works

  1. Type the organization, the meeting name, the date, the start and end time, and the location.
  2. Pick how many rows you need, from 1 to 200. Each row gets a number, and Name, Email, and Phone always print.
  3. Turn on any extra columns: First time?, Role or Affiliation, or Signature.
  4. Press Print. The sheet prints portrait in big type. A long sheet carries on to more pages with the headings printed again, and the last page ends with Recorded by and Total present lines.
  5. Want people to sign up ahead and get reminders? Turn it into a sign-up sheet. The meeting becomes one slot with a spot for each row, and reminders go out 24 hours and 1 hour before the slot's start time.

FAQ

How many rows can one sheet have?

Up to 200, and the sheet starts with 25. A portrait page holds about 18 to 23 rows, so the 25-row sample prints on two pages. A longer sheet carries on to more pages, with the meeting name and column headings printed again at the top of each one.

What do the extra columns do?

First time? gives newcomers a box to check, so you know who to welcome. Role or Affiliation fits a title, a grade, a company, or a member type. Signature adds a wide box for meetings that need a signed record, like trainings and board meetings.

What prints at the bottom of the sheet?

A Recorded by line for the person who keeps the sheet, and a Total present line to fill in after the meeting, so the count stays on paper with the names.

What happens when I turn it into a sign-up sheet?

SignUpSpark opens a new sheet with one Attending slot at your meeting time and a spot for each row, so people can say they're coming. You can change any of it before you publish. Reminders go out 24 hours and 1 hour before the slot's start time.

Is it free?

Yes. The printable sheet is free with no account. The Free plan includes 1 active event and 100 signups per event, so an online sheet with more than 100 spots needs Pro. Pro is $8.25 a month billed yearly ($99), or $12 month to month, with a 14-day free trial. See Pro pricing
